3. Damaged R.O.C. driving license (its submission is not needed if it is lost.).

4. Two 1-inch black-and-white photos or color photos with gloss surface, plain background, facial features showing clearly, no hat worn, frontal bust and taken within the previous six months. A processed digital photo will not be accepted.
